To make an appointment please call us at (423) 805-2514.  Our telephone is answered by a live answering service 24 hours a day, 7 days per week.  Rest assured that any call or inquiry is completely confidential. 

 

Please be prepared to provide the name of your insurance company if you want to use your medical benefit.  (We request this information to ensure that we are current providers and to assist with referrals in the unlikely event we do not participate in your network.)

 

Cancellations

Appointments must be canceled at least 24 hours in advance to avoid a late cancellation fee.   Please leave cancellation notices with our answering service at (423) 805-2514. 

 

All cancellations received within less than 24 hours in advance and all “no-shows” will be charged the late cancellation fee of $50.00.  THERE ARE NO EXCEPTIONS. Please respect that all appointments are reserved especially for you and cannot be utilized by others in need UNLESS you give us proper notice of cancellation.

 

Insurance/Payment Arrangements

Our clinicians support access to quality healthcare services for all.  Therefore we participate in most insurance plans as well as many employee assistance programs. (Please check with your carrier and clinician for participation)

 

We understand that individuals may not have insurance coverage or may choose to pay “out of pocket” for care due to high deductibles, limits in coverage, concerns about privacy, etc.  Hence, our office provides some of the most reasonable self-pay rates available.  We are happy to discuss any payment concerns with prospective and current clients.
